Job Description

Job Summary:

The Enrollment Case Manager (ECM) is responsible for conducting intake visits for Home and Community-Based Services (HCBS) waiver applicants primarily in their places of residence. The role involves facilitating the eligibility determination process, assisting applicants throughout the enrollment journey, and collaborating with various community stakeholders to improve access and remove barriers to HCBS programs. The ECM must be able to travel extensively within assigned and neighboring counties and maintain strong relationships with service providers and community organizations.

Responsibilities:
  • Travel throughout assigned and neighboring counties to conduct applicant intake visits in their residences.
  • Facilitate the eligibility determination process for HCBS waiver applicants from identification to enrollment or termination.
  • Present and explain HCBS program concepts, advising applicants of their rights and responsibilities.
  • Assist with application submission, including data entry of the Compass Application.
  • Support applicants in selecting Managed Care Organizations (MCOs), Life Providers, or Support Coordinators while ensuring consumer control.
  • Develop and maintain relationships with community-based organizations, advocates, and stakeholders to improve the application process.
  • Collaborate with physicians, County Assistance Offices, providers, nursing homes, and rehabilitation facilities serving mutual consumers.
  • Monitor and facilitate stalled cases to ensure timely processing.
  • Attend regularly scheduled meetings and report schedule availability as required.
  • Occasionally lift or move items weighing up to 25 pounds.
  • Perform other duties as assigned by management.
Required Skills & Certifications:
  • Minimum one year of experience in public or private social work.
  • At least two years of casework experience, including one year assessing individuals' functional ability for institutional or community-based services.
  • Bachelor's degree or equivalent combination of experience/training with at least 12 semester hours in sociology, social welfare, psychology, gerontology, or related social science.
  • Must have a valid personal vehicle; public transportation is not permitted.
Preferred Skills & Certifications:
  • Degree majoring in social welfare or related social sciences.
  • Experience working with HCBS waiver programs.
  • Strong interpersonal and communication skills for effective stakeholder engagement.
Special Considerations:
  • Extensive travel required within multiple counties, including Dauphin County, Pennsylvania.
  • Must have reliable personal transportation; public transit use is prohibited.
  • Occasional lifting of items up to 25 pounds.
  • Fixed interview and start dates.
Scheduling:
  • Work schedule involves traveling and conducting visits primarily during regular business hours.
  • Weekly salary and gas reimbursement payments provided, including a mileage bonus of $0.70 per mile.
